fix(eve): defer context during tool approval

What

• defer channel context together with follow-up text while a tool approval resolves
• replay that input on the next harness step
• preserve the Anthropic cache prefix across the approval and replay calls
• cover both the pending-input state and the two-call model sequence

Failure boundary

A Linear approval includes fresh <linear_context>. The harness appended that context after the tool approval response, but the AI SDK collects approvals from the tail tool message. The model call therefore missed the approval and the session ended with the original tool call still pending.

The approval response now remains the final message for its model call. No channel input is discarded; the deferred context is consumed on the next internal step. That second call retains the first call as an unchanged prefix and advances the Anthropic cache marker to the new context tail.
