feat(eve): group agent summary channels by authored channel (schema v4)

Description

The Vercel agent summary (.eve/agent-summary.json) emitted one flat channel entry per HTTP route, so a single authored channel appeared several times under the same name — e.g. channels/eve.ts (info/session/follow-up/stream routes) produced up to four entries all named eve, and the dashboard's deployment card rendered "5 Channels" with a tooltip of eve, eve, eve, eve for an agent with two authored channels (see vercel/front#76863, which fixes the display side for existing v3 artifacts).

Per-route granularity is right for the compiled manifest (the runtime mounts one handler per (method, urlPath)), but the summary's contract is the authored channel. This bumps the summary to schemaVersion 4: one entry per authored channel — name, display type, adapterKind, logicalPath — with route detail nested under a routes: [{ method, urlPath }] array, preserving first-occurrence order of both channels and routes. No information is lost.

Consumer impact: the dashboard proxies the artifact without schema validation, and after vercel/front#76863 the card only reads name/type (both still top-level), so v4 artifacts render correctly there as-is. Front's AshChannelEntry type can pick up the nested-routes shape in a follow-up if the dashboard wants to display endpoints.

How did you test your changes?

• pnpm --filter eve exec vitest run --config vitest.unit.config.ts src/internal/nitro/host/build-vercel-agent-summary.test.ts — updated the grouping assertions and added a multi-route channel to the fixture (4/4 passing)
• pnpm fmt, pnpm lint, pnpm typecheck — clean
