fix(eve): keep the dev instrumentation entrypoint stable

Summary

Nitro retains its plugin list for the lifetime of eve dev. When authored instrumentation existed at startup and was later removed, Nitro still retained that plugin path; deleting the generated plugin or leaving it importing the removed source could make a later structural rebuild fail with an unresolved import.

This change always generates and registers a stable instrumentation entrypoint, rewriting it to a no-op when instrumentation is absent. Instrumentation add, edit, and remove events trigger a structural reload, and every supported instrumentation extension remains marked as side-effectful.
