Introduce Bootstrap v2 operational onboarding

This PR separates required instance activation from resumable operational onboarding.

Highlights:

• Reduces /bootstrap to a short required activation flow
• Adds section-based operational onboarding states:
  • not_started
  • in_progress
  • ready
  • skipped
• Adds an operational onboarding checklist in admin setup
• Keeps skipped sections resumable and distinct from completed sections
• Prevents real clean-instance activation from inheriting fictional demo operational records
• Preserves explicit demo and restore modes
• Adds Bootstrap v2 architecture and rehearsal documentation
• Uses the existing persistence boundary without direct Drizzle calls
