docs: record stalled Firstmate lane recovery

Intent

Recover five stranded Firstmate branches after their workers were killed, without discarding work. Inventory every lane before mutation; preserve and publish all real uncommitted and disk-only commits; explicitly classify rollback scratch before cleaning it; open or confirm pull requests for review; deliberately close the detached rules lane when its exact head is already in an open PR; never merge, force-push, rewrite gate-tracked history, drop a stash, or run the full local test suite. Ship a durable short recovery report through review, lint, push, PR, and GitHub CI, skipping only the unsafe local full-test step.
