Apply the lesnik512/planning-convention APPLY.md 2.0.0 update: swap index.py + templates and drop the plan.md template, flatten the two change folders to flat change files and drop their committed plan.md, merge the 2.0.0 convention prose into planning/README.md, retire "bundle" wording in CLAUDE.md/justfile, and record .convention-version=2.0.0.
