docs: repoint pre-commit agent rule links to the on-demand skill#69

The always-loaded agentsmd/rules/{pre-commit,nix-tool-policy,
shared-workflow-org-refs}.md files migrated to on-demand skills in
claude-code-plugins v4.2.0; ai-assistant-instructions deletes them
in its Phase 2b PR. Repoint the two deep links and update the
ownership description.

Companion to dryvist/ai-assistant-instructions#682 (Phase 2b of #680's consolidation). The three situational rules became on-demand skills in claude-code-plugins v4.2.0, so:
