New parameter: extraction_mode ("balanced" or "accurate"). Controls
the extraction pipeline independently from mode (which controls
parsing). When not set, the API defaults to balanced.
Accurate mode returns per-field verification metadata including
extraction_status, reasoning, citations, and verification feedback.
