Catch lambda timeouts

Error matching in API Gateway was broken when errors in Lambda didn't originate from the lambda function

Documentation fixes/clarification

• Added S3_BUCKET to deploy container so it doesn't need re-exported
• Fixed issue if no lambda functions exist in an AWS account

Deployment fixes

• Support federated users with AWS_SECURITY_TOKEN and AWS_SESSION_TOKEN
• Don't lock function upload to us-west-2
