Stop describing the PR comment as immutable
When this ledger is later used as an evidence source, an ordinary GitHub PR comment can still be edited or deleted by its author and deleted by repository moderators, so calling it “immutable” overstates the integrity of the recorded review. Preserve a content-addressed snapshot if immutability is required, or describe this only as a timestamped root-cause comment.
