# Multi-scale, multi-modal imaging assessment of trajectories of cognitive impairment in Multiple Sclerosis

## Dataset Overview

This dataset contains longitudinal, multi-modal MRI data collected as part of a study examining the neural mechanisms underlying cognitive impairment in people with multiple sclerosis (MS). The study employs a multi-scale, multi-modal imaging approach to characterize macroscopic brain network changes, assessed via functional and structural connectivity, alongside microscopic neuronal integrity markers. The goal is to identify patients at risk for cognitive decline and informing the development of personalized, neurobiologically-based therapies.

## Study Description

The full study will include longitudinal scanning over approximately 4 years in 66 participants with MS and 30 healthy controls. This dataset represents the first release, containing baseline (`ses-1`) data for 20 MS patients and 8 healthy controls. Additional sessions and participants will be added as data collection proceeds.

## MRI Scanner

All data were acquired on a **3 Tesla Siemens Prisma** scanner (Prisma_fit, syngo MR E11) at Citigroup Biomedical Imaging Center, Weill Cornell Medicine, using a 64-channel head/neck coil.

## Dataset Contents

This BIDS dataset includes the following modalities for each participant and session:

### Anatomical (anat/)

**T1-weighted MPRAGE** - 3D sagittal acquisition for cortical parcellation, gray matter volumetry, and image registration.

- Resolution: 0.8 mm isotropic
- Matrix: 320 x 320
- TR / TE / TI: 2400 / 3.14 / 900 ms
- Flip angle: 9°
- GRAPPA acceleration: 2x
- De-faced using FreeSurfer MiDeFace

### Functional MRI (func/)

**Multi-echo resting-state fMRI (BOLD)** - acquired using the CMRR multiband EPI sequence (`cmrr_mbep2d_bold`) with 5 echoes. Each subject has up to 2 runs (`run-1` and `run-2`); 2 subjects (sub-1567, sub-9001) have only `run-1`. Single-band reference (sbref) images are included for each echo and run.

- Echo times (TE): 13.4, 31.1, 48.8, 66.5, 84.2 ms
- TR: 1.355 s
- Multiband acceleration factor: 6
- In-plane GRAPPA: 2x
- Flip angle: 68°
- Voxel size: 2.4 mm isotropic
- Slices: 72
- Matrix: 90 x 90
- Run duration: ~867 s (640 volumes per run)
- Task: resting-state (eyes open)

Multi-echo data should be preprocessed with a ME-ICA-compatible pipeline (e.g., [tedana](https://tedana.readthedocs.io)) prior to functional connectivity analysis.

### Field Maps (fmap/)

**Spin-echo EPI fieldmap pairs (AP/PA)** - blip-up/blip-down acquisitions for 
susceptibility distortion correction of the multi-echo fMRI data. Two pairs are 
present per session (run-1 and run-2), with `IntendedFor` fields in the JSON 
sidecars linking each pair to its corresponding fMRI run.

- Sequence: `SpinEchoFieldMap_AP` / `SpinEchoFieldMap_PA`
- Voxel size: 2.4 mm isotropic
- Slices: 72
- TE: 75 ms; TR: 7.08 s

### Diffusion-Weighted Imaging (dwi/)

**Multi-shell DWI** - based on the HCP-Lifespan protocol, acquired in both AP and PA phase-encoding directions (2 runs per direction). 3 subjects (sub-0858, sub-1985, and sub-9005) have only `run-1`. Refer to the `.bval` and `.bvec` files for the complete gradient scheme.

- Sequence: `DWI_Harm_98Dir_InvPE-Off` (`cmrr_mbep2d_diff`)
- b-values: 0, 1500, 3000 s/mm²
- Directions: ~92 per shell (98 total directions per run including b=0)
- Phase-encoding: AP and PA (with `run-1` and `run-2` for each)
- Voxel size: 1.5 mm isotropic
- Multiband acceleration: 4x
- TE / TR: 89.2 / 3230 ms

### Perfusion (perf/)

**Pseudo-continuous ASL (PCASL)** - 3D Turbo Gradient Spin Echo (tgse) PCASL sequence (`tgse_pcasl_ve11e`) for quantification of cerebral blood flow (CBF). Each session consists of 11 control/label pairs (22 volumes total), stored in acquisition order and described by an accompanying `_aslcontext.tsv`. No separate M0 scan was acquired; CBF quantification should use the control images as an M0 approximation (`M0Type: Absent`).

- Labeling type: PCASL
- Labeling duration: 700 ms
- Post-labeling delay (PLD): 2000 ms (single-PLD)
- Background suppression: on
- Total acquired pairs: 11 (22 volumes)
- Acquired voxel size: 3.75 mm isotropic (reconstructed to 1.875 x 1.875 x 2.5 mm via scanner interpolation)
- Matrix: 64 x 64 acquired, reconstructed to 128 x 128
- Slices: 48
- FOV: 240 mm
- TR / TE: 4000 / 35.3 ms
- Flip angle: 120°
- 3D EPI readout (EPI factor 57, turbo factor 14)

Group assignments (`patient` / `control`) are provided in `participants.tsv`. This table will be updated as additional participants and sessions are released.

## Sessions

This release contains only baseline data (`ses-1`) for each participant. Longitudinal follow-up sessions will be added as data collection proceeds over the course of the approximately 4-year study.

## Notes on Missing Data

- sub-1567 and sub-9001 have only one fMRI run (`run-1`). All other participants have two runs.
- sub-0858, sub-1985, and sub-9005 have only half the diffusion data (`run-1`). These contain both AP and PA and all shells, but only half the diffusion directions.
- PCASL data are absent for sub-1567 and sub-9005
