Update docs related to MCP v2 compatibility.
Improve reliability of the MCP tools for some corner cases; mainly happen when MCP service does not match graph version, hence differences in the schema/model. DB and MCP version matching will be improved in follow up PRs, but this PR improves so to avoid MCP exceptions.
