Description
Problem-capture flows write unverified root-cause mechanism claims as established fact, and the false claims propagate. Driver: a ticket captured with "our wrapper today drops everything except the row-level summary" asserted as declarative fact; the first live call the same day disproved it (the fields were fully present in the deployed response). By then the false mechanism had been replicated into 5 places in the ticket body, requiring 5 corrective edits, and had nearly steered the fix work at a non-existent problem.
Symptoms
- Capture-time mechanism claims that were never executed/observed are written as fact rather than as "hypothesis — unverified" with a verification task.
- Asserted-fact capture propagates through ticket bodies and biases the subsequent investigation toward confirming rather than testing the claim.
