One of my local rivers, the river keepers made a map which notes quite a few hazards.
It kind of has two levels of severity
Severity
- Advisory (an indication to pay attention to)
- Impassable (or seasonally impassable)
Hazard
- Persistent Log Jam
- Seasonal hazards like areas shallow in summer (where there is one seasonally impassable shallows, and one advisory shallows)
- Dams without safe portage
I'm not sure if there exist tags for this kind of thing. If not it'd be nice to try and come up with some standard tags for them.

The hazard=* tag is intended to tag hazards that are explicitly declared by posted signage and/or government declaration. Consistent with OSM conventions, mappers should only tag hazard features that are permanent or recurring, rather than temporary.

These hazards rarely have such official signage.
