Implemented the scoped entry-optimization pieces in PR #104:

• added command-palette markdown insertions for 2x2 and 3x3 tables
• added bullet list and numbered list insertions
• added fenced code block insertion with useful cursor placement
• improved command-palette grouping/discoverability by moving layout actions under workspace and documenting markdown inserts in Help/Quickstart

The LLM-generated markdown idea is intentionally left out of this issue because it needs a separate provider/privacy/API-key design.
