Script not running

Hi Guys,

Thanks for this form, I followed your walkthrough and am really pleased with how it's looking.

One minor issue. I'm not massively proficient in .php, and when I test the page the script doesn't seem to run. When I hit the 'Send' button it redirects to a page showing the script. I'm really confused!

Can you suggest where I am going wrong? I've attached some screenshots.

Your just have to change the $to = 'example@domain.com'; to the email you would like the information sent to and save upload and you should be all set.

Thanks Santos - I'd changed that after posting - forgot I was just reposting the actual script. My issue was that I wasn't running it on a server. Got WAMP and it's done the trick.
The email's not sent to the inbox specified, but again, this is possibly a software issue as opposed to the code.
