Consolidate setup scripts into single setup.sh

- Add interactive terraform init/apply to setup.sh
- Add ACR image build and container app update flow
- Show deployment URLs and useful commands after completion
- Remove scripts/setup-azure-deployment.sh (pointed to wrong terraform path)

Now there's only one setup script that handles the full deployment flow.
