Make the feedback form clean, professional, and accessible. Add two-column Ratings (stacks on mobile), clear focus states, inline date validation, phone input filter, and a toast on submit. Keyboard-friendly star ratings with ARIA.

Acceptance Criteria

• Ratings grid: 2 columns ≥760px, 1 column on mobile
• Consistent spacing + visible focus rings
• Date “To” ≥ “From”; phone allows digits/space/+/-
• Toast replaces alert; star ratings are keyboard accessible
