Failed to register repository to project — org repo (SPulse-Org/SPulse)

Getting "Failed to register repository to project" when trying to register SPulse-Org/SPulse to a campaign.

Payload being sent:

• github_repo_id: 1295729378
• github_url: https://github.com/SPulse-Org/SPulse
• name: SPulse

Steps tried:

• Uninstalled and reinstalled GrantFox OSS GitHub App
• Verified app is installed on SPulse-Org organization with access to the repo
• Error persists every time

Is there a known issue with registering org repos?
Is SPulse-Org/SPulse already registered to another project?

Hitting the same error registering Payhub-protocol/payhub — different org, same symptom.

• GrantFox OSS GitHub App confirmed installed on the org (repository_selection: all, verified via API)
• Error: "Failed to register repository to project"

Two independent orgs hitting this now — seems like a platform-side bug in org-repo registration, not an app-permission issue on our end.
