User avatar / profile image is not displayed in the header

The icon at the top right looks like it should display the user's avatar, but it always displays a generic user profile icon.

See screenshot, showing my avatar correctly in the history log but not at all in the Studio header.

Note: this seems to be a broader issue, affecting the LMS too?

The PRs mentioned do not address the new Instructor Dash MFE (which pulls the header in from frontend-base but seems to have the same issue) or the LMS (which doesn't show an icon at all). I'll look into both when I have a moment.
