Copy/paste shortcuts broken on VSCode Extension

Description

On MacOS with latest VSCode and OpenCode Beta extension, Cmd-V and Cmd-C do not work to copy paste. To get output copied to the clipboard, you have to go to the VSCode edit menu and click "copy."

Steps to reproduce

Create new assistant session (e.g. via "OC" button), try to paste something from the clipboard into the prompt field by pressing cmd-V, observe it fails.
